Care Coordination

Care Coordination is intensive case management, advocacy, information, and referral services for children and families.

Care Coordination services are available to children and adolescents that meet the following criteria:

Have serious emotional and behavioral disturbances that require multiple agency coordination
Are at risk for being seperated from their families and communities
Are currently in an out-of-home placement and need service coordination to return home

The Care Coordinator works with the family to identify the child's strengths and needs, and a team is developed to create an individualized service plan.
